Advance Care Planning Resources

The Conversation Project Coaches – specially trained volunteers will help you assess your values and wishes and prepare for a conversation with your family and your health care provider. They will also help you complete the Medical Durable Power of Attorney document. This is a free service. www.theconversationprojectinboulder.org or 303-422.0436, ext 133.

There are other professionals in our community who offer different or more in-depth advance care planning support. There is a fee for their services but some will work on a sliding fee scale. We do not endorse their services but offer them as an additional resource. Some medical offices, attorneys and faith communities also offer support with advance care planning.
